Patient Terms of Use
1. About These Terms
These Patient Terms of Use ("Patient Terms") apply when you access the FinVerified patient session portal (the "Patient Portal") via a Magic Link sent by your dental practice. The Patient Portal is operated by FinVerified, Inc. on behalf of your practice. By accessing the Patient Portal, you agree to these Patient Terms.
These Patient Terms supplement the full Terms of Service and apply specifically to patient-facing features.
2. What the Patient Portal Does
The Patient Portal allows you to:
- Review your treatment cost estimate and financing disclosure documents
- Review available financing options presented by your practice
- Electronically acknowledge and sign payment authorization and consent documents
- Access your disclosure records for the session associated with your Magic Link
The Patient Portal does not originate financing, make credit decisions, or represent any lender. Financing decisions are made entirely by the lender shown in your disclosure documents.
3. Magic Link Access
Access to the Patient Portal is provided through a time-limited Magic Link sent to you by your dental practice. Your Magic Link is personal and should not be shared. FinVerified is not responsible for unauthorized use resulting from your sharing of the link. Links expire after 7 days; contact your practice if you need a new link.
4. Your Information
FinVerified handles your information in accordance with our Privacy Policy and, where applicable, our HIPAA Notice of Privacy Practices. We do not sell your personal information. Your practice remains the primary data controller for your healthcare information.
5. Financing Disclosures
Any financing options shown in the Patient Portal are provided for informational purposes. All financing terms, rates, and conditions are determined solely by the lender, not by FinVerified or your practice. You are not required to accept any financing option. Your treatment is not conditional on financing acceptance. FinVerified does not receive compensation based on your financing decisions.
6. Electronic Signatures
By checking an acknowledgment box or clicking "I agree" in the Patient Portal, you are providing an electronic signature that has the same legal effect as a handwritten signature under the Electronic Signatures in Global and National Commerce Act (E-SIGN) and applicable state law. Please read all documents carefully before signing.
